I hereby certify that the facts outlined in the above employment application are accurate and complete to the best of my knowledge and authorize Stella May Contracting to verify their accuracy and obtain reference information on my work performance. I now release Stella May Contracting from any/all liability of whatever kind and nature, which, at any time, could result from obtaining and having an employment decision based on such information.

I understand that, if employed, falsified statements of any kind or omissions of facts called for on this application shall be considered a sufficient basis for dismissal.

I understand that should an employment offer be extended to me and accepted, I will fully adhere to the Employer's policies, rules, and employment regulations. I further understand that neither the policies, rules, regulations of employment, or anything said during the interview process shall be deemed to constitute the terms of an implied employment contract. I understand that any employment offered is for an indefinite duration and at will. The Employer or I may terminate my employment at any time with or without notice or cause.

I hereby attest that should any dispute between Employee and Employer arise at any time out of any aspect of the employment relationship, including, but not limited to, the hiring, performance, or termination of employment and/or cessation of employment with the Employer and/or against any employee, officer, alleged agent, director, affiliate, subsidiary or sister company relationship, or relating to an application or candidacy for employment, Employee and Employer will confer in good faith to resolve promptly such dispute. In the event that Employer and Employee are unable to resolve their dispute and should either desire to pursue a claim against the other party, both Employer and Employee agree to have the dispute resolved by final and binding Arbitration. The Employee and Employer agree that the Arbitration shall be held in the county and state where Employee currently works for Employer or most recently worked for Employer.

I consent to undergo substance abuse testing and state and federal background checks if offered a position for employment.
